Terms and Conditions

Liability Waiver

To the best of my knowledge, I am in good physical condition and able to fully participate in this Mindfulness Program, Flash Mob and Choir, Coaching, Retreat, Workshop, Qigong Tai Chi class or any other SolJoy with Anna Choi SPC event, program, or training. I am fully aware of the risks and hazards connected with the participation in Qigong, Tai Chi, Yoga, Flash Mob, or any other physical program including physical injury or even death, and hereby elect to voluntarily participate in said event, knowing that the associated physical activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or loss of damage to property owned by me, as a result of participation in this course. I further certify that I am at least 18 years of age. If under 18, my parent/guardian is the below signed. 

I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, SolJoy, Anna Choi their officers, servants, agents, and employees as well as Island Fitness and their staff (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury including death, that may be sustained by me, or to any property belonging to me, while participating in physical activity, or while on or upon the premises where the event is being conducted. 

It is my expressed intent that this release and hold harmless agreement shall bind the members of my family and souse, if I am alive, and my heirs, assigns, and personal representative, if I am deceased, and shall be deemed as a RELEASE, WAIVE, DISCHARGE, AND CONVENTION TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in accordance with the laws of the state of Washington. 

In signing this release, I acknowledge and represent that I HAVE READ THE FOREGOING Waiver of Liability and Hold Harmless Agreement, UNDERSTAND IT AND SIGN IT VOLUNTARILY as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreements have been made; and I EXECUTE THIS RELEASE FOR FULL, ADEQUATE, AND COMPLETE CONSIDERATION FULLY INTENDING TO BE BOUND BY SAME. 

Photo and Video Release Waiver

I hereby grant SolJoy with Anna Choi SPC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the SolJoy with Anna Choi SPC and will not be returned.

I hereby irrevocably authorize the SolJoy with Anna Choi SPC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge the SolJoy with Anna Choi SPC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY ELECTRONIC SIGNATURE IN CLICKING ACCEPTANCE OF TERMS OF CONDITIONS.
